NDIS Orders

For all NDIS orders, please send us an email at hello@medforte.com.au with:

1. Your Full Name;
2. Your NDIS Client Number;
3. Your Billing Address;
4. Your Shipping Address (if different from your billing address); and
5. Your order (we also offer spare 28 day carousel trays at $29 each).

We will prepare an invoice for you to submit to your Plan Manager. Once payment is received, we will dispatch your order.
